چکیده

In impaction grafting of contained bone defects after revision joint arthroplasty the graft behaves as a friable aggregate and its resistance to complex forces depends on grading, normal load and compaction. Bone mills in current use produce a distribution of particle sizes more uniform than is desirable for maximising resistance to shear stresses. We have performed experiments in vitro using morsellised allograft bone from the femoral head which have shown that its mechanical properties improve with increasing normal load and with increasing shear strains (strain hardening). The mechanical strength also increases with increasing compaction energy, and with the addition of bioglass particles to make good the deficiency in small and very small fragments. Donor femoral heads may be milled while frozen without affecting the profile of the particle size. Osteoporotic femoral heads provide a similar grading of sizes, although fewer particles are obtained from each specimen. Our findings have implications for current practice and for the future development of materials and techniques.

Tricalcium-phosphate and hydroxyapatite bone-graft extender for use in impaction grafting revision surgery. An in vitro study on human femora.

Impacted morsellised allografts have been used successfully to address the problem of poor bone stock in revision surgery. However, there are concerns about the transmission of pathogens, the high cost and the shortage of supply of donor bone. Bone-graft extenders, such as tricalcium phosphate (TCP) and hydroxyapatite (HA), have been developed to minimise the use of donor bone. In a human cadav...

Impaction bone-grafting in revision joint replacement surgery.

The standard graft material for impaction bone-grafting is fresh-frozen femoral head allograft morselized to a particle size as large as is practical to ensure stability and allow new bone formation. The graft must be sufficiently compacted to provide immediate mechanical stability; this requires containment of the graft and substantial impaction energy. Diaphyseal bone fracture and excessive i...

Cells seeded on MBG scaffold survive impaction grafting technique: Potential application of cell-seeded biomaterials for revision arthroplasty.

The objective of this study was to investigate the potential application of cell-seeded biomaterials for revision arthroplasty and the reconstruction of major joints using the impaction grafting technique. Using morselized cancellous bone graft as a porous scaffold, MG63 cells were seeded on the scaffold and impacted into an acetabulum cup model using a mechanical device constructed from data o...

Effect of Gamma Irradiation on the Mechanical Properties of Morselised Allograft

INTRODUCTION: Impaction grafting is a common procedure in the reconstruction of cavity defects. Concerns regarding sterility of the graft material may require gamma irradiation prior to implantation. The grafts’ mechanical properties play an important role in surgical handling and in-vivo response. Gamma irradiation adversely affects the mechanical properties in a dose dependent fashion in cort...

Time-dependent mechanical properties of HA/TCP particles in relation to morsellized bone grafts for use in impaction grafting.

In reconstructive surgery human bone defects are sometimes filled with the use of the impaction bone grafting technique. Currently different types of biomaterial particles are being developed as bone-substitute materials. Before these biomaterials can be applied their mechanical and biological behavior should be characterized. In this study the time-dependent mechanical behavior of biomaterial ...
